Notes for using this form:    Select your home club from the drop down list.    Select the away club from the drop down list. The prompt “dd/mm/yy” will clear when you click on the field.  Enter the match date in the same format.  The prompt  “Enter your name” will clear when you click on this field and enter your name.  You can edit your entries to correct them before submission or clear the form to the pre-set values by pressing “RESET”.   When you are satisfied that the information is correct press “SUBMIT”.
