Notes for using this form. Select the home club using the drop down menu. Select the away club using the drop down menu. Select the score using form the drop down menu and enter the date the match was played “dd/mm/yy” as shown on the prompt in the field. The prompt dd/mm/yy will disappear when you click on the field. Select your name from the drop down menu and select the your club from the drop down menu. You may edit your entry in each field or press the “RESET” button to return the form to it’s original state. When you are sure the information is correct press the “SUBMIT” button.
